Davies for their assistance in studying the cases, and Dr. Shepherd Ivory Franz for his advice and criticism in preparing this thesis. Dr. Lewis furnished the reports of the physical examinations which are included in the case histories. The names of the patients are fictitious. In quoting their talk I have often translated obscene words into more technical language but I have not altered the meaning in any way.
